Responsibilities:

Plan and implement individual patient training strategies (Home Hemo, and Peritoneal Dialysis).

Family and environmental (home) assessments

Contributes to interdisciplinary team initiatives.

Monitors and reevaluates patient progress to ensure quality outcomes.

Acts in the role of charge nurse as assigned.

 

Experience and Education:

Completion of a nursing program from an accredited school of nursing.

Current license to practice as a registered nurse in the State of Michigan and/or Ohio.

Have 6 to 12 months experience as an RN, including at least 6 months of experience providing nursing care to Home/PD dialysis patients prior to assuming charge role
