PATIENT PLACEMENT COORDINATOR

Are you  interested in promoting our GHS programs and services through an expanded network of physicians, unit and administrative staff, discharge planners, and internal and external customers? The challenge of appropriately placing a new dialysis patient in an assigned unit involves harmonious communication with patients, families, physicians, administrative and clinical staff, and medical insurance providers. Are you up to the challenge?

 

Responsibilities:

Communicates with HFHS and GHS medical and nursing staff to facilitate a smooth transition of patients to the out-patient dialysis setting.

Facilitates activities for timely discharge planning, through efficient transfer of patient medical information and patient placement in assigned units

Provide monthly reports on new referrals, transient dialysis, and payor sources.

Communicates with insurance companies, community organizations, clinical and support staff, and functions as a patient advocate when necessary.

 

Experience and Education:

Masters of Social Work degree required.

Computer skills sufficient to perform word processing, Excel, and HFHS mainframe.

Oral, written and presentation skills required.
