Click on the INCIDENT BUTTON BELOW. A blank Incident Form will download to your computer. Fill it out, then Click INITIAL SUBMIT at the top of the form.

You will be prompted to SAVE the form on your computer. Edit the filename to indicate the location and date in the name. Then you will be prompted to locate the saved form on your computer. It will be sent to the Incident Server.
